About the role
Lakefront Nursing and Rehabilitation Center, located in the Rogers Park neighborhood of Chicago, is seeking a Licensed Practical Nurse (LPN) to join our clinical team. For over 40 years, we have provided high-quality skilled nursing care, rehabilitation, and mental health programming in a community-focused environment. Our facility emphasizes community integration, helping residents—including those dealing with addictions and other mental health conditions—reintegrate into independent living through comprehensive programming and collaboration with agencies throughout Chicago.
Responsibilities
- Ensure residents receive medication and nursing care as directed by their individual care plan
- Conduct resident assessments
- Serve as a team leader within the Clinical Department
- Advocate for residents to ensure their comfort and safety
Qualifications
- Unencumbered State of Illinois Licensed Practical Nurse (LPN) license
- Prior LPN experience is preferable, but we are willing to invest in the right candidate
