Licensed Practical Nurse (LPN)
About the role
The Licensed Practical Nurse (LPN) at CMHS, Fort Carson, CO, will provide direct patient care under the supervision of physicians and RNs. They will perform patient assessments, document vital signs, administer medications, and update patient records. Additionally, they will support the Referral Management Team by processing, tracking, and coordinating specialty care referrals. The LPN will educate patients and families on treatment plans, medication use, and health benefits. They will also assist in analyzing referral and utilization data, preparing reports, and ensuring compliance with TRICARE and DHA policies.
Responsibilities
- Provide direct patient care under the supervision of physicians and Registered Nurses (RNs).
- Perform patient assessments, document vital signs, administer medications, and update patient records.
- Support the Referral Management Team by processing, tracking, and coordinating specialty care referrals.
- Educate patients and families on treatment plans, medication use, and health benefits.
- Aid in analyzing referral and utilization data, preparing reports, and ensuring compliance with TRICARE and DHA policies.
- Maintain accurate nursing and referral records, ensuring timely data entry into EHR systems.
- Demonstrate knowledge of utilization management and review (UM/UR) principles when supporting referral workflows.
- Provide consultation and administrative support for TRICARE beneficiaries.
Requirements
- Education: Graduate of a practical nursing program accredited by a U.S. agency.
- Experience: Minimum of 3 years of clinical nursing experience within the last 5 years. Experience in referral management, TRICARE systems, or military healthcare environments highly preferred.
- License/Certification: Current, full, active, and unrestricted LPN license in a U.S. jurisdiction; BLS certification required.
- Other Requirements: Must be a U.S. Citizen. Proficiency with Microsoft Office (Word, Excel, Access) and EHR systems (GENESIS preferred). Strong communication, organizational, and customer service skills. Knowledge of patient privacy, HIPAA regulations, and medical ethics.

Tentative starting salary is based on experience and qualifications.
Schedule
Monday–Friday, 8-hour shifts between 0600–1700, excluding federal holidays.
