Licensed Practical Nurse (LPN)

Job Summary

Pioneers in the home care industry, Interim HealthCare® is seeking Home Health LPNs who can provide exceptional patient care while enjoying flexible scheduling and work-life balance.

What You’ll Do

• Provide home-based care to patients with illnesses, injuries, and chronic diseases
• Work with a team of physicians, RNs, CNAs, and aides to deliver quality patient care
• Take patient vitals, perform assessments, document progress, and report changes to RN supervisor
• Assist RN in implementing the patient’s plan of care and teaching self-care techniques
• Educate family members on the patient’s plan of care, medications, and home safety
• Collaborate with interdisciplinary team to coordinate patient care and plan for discharge

Requirements

• Graduate of an accredited practical nursing program and active LPN license in Montana
• 1 year of LPN experience, preferably in home healthcare
• CPR certification
• Knowledge of state and federal home health regulations
• Strong clinical proficiency in nursing, excellent communication skills, and compassion
