Licensed Practical Nurse (LPN)

About the role

As a member of the Hospice team, the LPN works primarily in a support role by performing tasks and responsibilities within the framework of case findings, health teaching, health counseling, and provision of supportive care under the direction of a registered nurse carrying out the nursing process. The LPN works under the direct supervision of the RN or licensed physician and interacts with patients, spouses, significant others, family members, physicians, and fellow team members in keeping with the mission and mandate of Hospice.

Responsibilities

• Provides services within the boundaries of the State Practice Act and responsibilities listed in the Department of Patient Services.
• Provides input (including any changes in condition) and supports the decisions and direction of the team as outlined in the hospice plan of care and as directed by the primary RN Case Manager.
• Makes observations and collects data both by interview and physical assessment to identify needs and/or problems and selection of standards.
• Provides anticipatory guidance and support to the patient/family, according to the interdisciplinary plan of care and as directed by the primary hospice RN Case Manager, Social Worker, Pastoral Care, and/or Coordinator of Patient Care.
• Records monitored information (i.e., I&O, VS).
• Participates in patient/family instruction and documentation of instruction and patient understanding on progress notes or teaching protocol.
• Communicates a daily report on each patient assigned to the patient’s case manager.
• Performs phlebotomy as needed.
• Performs ADLs when scheduled or needed.
• Responsible for hourly documentation on a Continuous Care Interdisciplinary Progress Note in the continuous care chart in the home or nursing home. Follows the hospice documentation policy when completing patient documentation.
• Responsible for notifying hospice immediately in the event the patient expires during a continuous care shift. The hospice primary RN or On-Call RN is responsible to attend a hospice death.

Requirements

• New York State licensed as a practical nurse.
• Minimum of two years’ experience in nursing – working in homecare or a hospice setting desirable.
• Attain Hospice & Palliative Licensed Nurse Certification after 500 hours or 1 year of employment.
• Requires flexibility to meet assigned tasks.
• Ability to work effectively and cooperatively as a team member.
• Capability to prioritize and handle multiple tasks efficiently.
• Highly developed organizational skills.
• Effective communication in English, both verbally (in person and by phone) and in writing.
• Bilingual is a plus.
• Valid Driver’s License and reliable transportation for travel to patient homes.
• Ability to carry up to twenty-five pounds of equipment and/or supplies.
• Agreeable to work in either county when needed.

Work Environment

• Work in various home environments and conditions.
• Various travel to patient homes using reliable transportation.
• All shift availability with monthly on-call weekend rotation.
• Moderate to high-risk of exposure to unsafe environments.

Schedule

Full Time: Tuesday–Saturday or Sunday–Thursday

• 8:30am–4:30pm or 12:00pm–8:00pm
• On-call rotation

Pay

$28–33 per hour (based on experience)
