Hospice of Orange & Sullivan Counties is a nonprofit provider of compassionate end‑of‑life care across the Hudson Valley. We deliver comfort, dignity, and support to patients and families in their homes, hospitals, nursing facilities, and at our Kaplan Family Hospice Residence.
About the Role
We are seeking a dedicated Licensed Practical Nurse (LPN) to join our Kaplan Family Hospice Residence (KFHR) Clinical Care Team. The position provides patient care under the guidance of a Registered Nurse, following each patient’s individualized Plan of Care. The LPN will work closely with the hospice interdisciplinary team to ensure high‑quality, patient‑centered care for residents of our 11‑bed residence located at 1 Sunrise Lane, Newburgh, NY 12550.
Responsibilities
- Provide direct patient care in accordance with the State Nurse Practice Act and the established Plan of Care under RN supervision.
- Accurately document care, patient condition, and pain/symptom management.
- Communicate patient status changes promptly to the Registered Nurse.
- Deliver effective pain and symptom management and evaluate patient response.
- Educate patients and families on medications, treatments, diet, and self‑care techniques.
- Collaborate with the interdisciplinary hospice team to ensure coordinated care.
- Adhere to infection control standards and all hospice policies and procedures.
Schedule
Three (3) twelve‑hour shifts per week, 7:00 am – 7:30 pm, including an every‑other‑weekend (Saturday & Sunday) commitment.
