Licensed Practical Nurse (LPN)

About the role

Homeview Health and Rehabilitation is seeking compassionate Licensed Practical Nurses (LPNs) for full-time evening shifts.

Responsibilities

• Provide daily care, health needs, safety, and comfort to residents.
• Conduct professional assessments and document residents' health.
• Assist with managing assigned nursing units' staff.
• Supervise, perform, and provide care assignments, routine skills, and procedures.
• Develop and implement resident care plans, maintain up-to-date care plans, and document progress.
• Maintain and revise care plans according to facility policy and procedure.
• Make daily and periodic visits to observe residents for symptoms, reactions to medication, or treatment.
• Obtain and document vital signs, intake and output, weights, diet supplements, and replacements, cultures, etc.
• Assist with the orientation and ongoing training of staff to maintain required skill levels.
• Report and document resident and/or visitor incidents, accidents, complaints, or allegations using the designated occurrence reporting system.

Qualifications

• A graduate of an accredited school of nursing with a valid LPN License in good standing according to State and Federal requirements.
• A thorough understanding of safe and effective nursing practices in the State of Indiana.
• Basic computer skills including proficiency in programs like Excel and Word.
• Personal integrity and the ability to interact tactfully with residents, family members, visitors, government agencies, and personnel.
