About the role

Join Our Team as a Floor Nurse (LPN)! Are you a licensed nurse with a heart for senior care and a commitment to clinical excellence? Frazee Care Center is looking for dedicated LPNs to provide compassionate, high-quality care to our residents.

Responsibilities

  • Deliver Compassionate, Clinical Care
  • Provide direct nursing care in accordance with resident care plans, physician orders, and community policies.
  • Administer medications and treatments accurately and on time.
  • Monitor resident conditions and intervene as needed to ensure health and safety.
  • Lead & Support The Care Team
  • Support CNAs and other direct care staff to ensure consistent, quality care.
  • Communicate changes in resident status to physicians, families, and interdisciplinary team members.
  • Participate in care planning, resident assessments, and quality improvement initiatives.
  • Foster a Safe & Supportive Environment
  • Uphold infection control, safety, and emergency response protocols at all times.
  • Promote dignity, comfort, and respect in all resident interactions.
  • Respond promptly to call lights and urgent care needs with a calm and professional presence.
  • Be a Clinical & Community Leader
  • Mentor new staff and share knowledge with team members.
  • Collaborate with families and other departments to meet holistic care goals.
  • Represent the nursing team positively in community events and care conferences.

Requirements

  • Licensure & Credentials
  • Active LPN license in the State of Minnesota.
  • CPR certification required (or willingness to obtain upon hire).

Qualifications

  • Previous experience in skilled nursing or senior living preferred, but not required.
  • Strong assessment, documentation, and communication skills.
  • Able to stay organized and calm under pressure while leading others.

Physical Requirements

  • Ability to lift, move, and assist residents as needed.
  • Must be able to stand, walk, bend, and perform physical tasks throughout a full shift.

Pay

$23.00 - 29.00 per Hour*

Schedule

$3.00/Hour PM Shift Differential

Schedule: 2:00 - 10:30pm* Every other weekend and every other holiday rotation required
