Licensed Practical Nurse-LPN

Responsibilities

• Perform ongoing nursing assessments and monitor residents' health and well-being
• Supervise, mentor, and guide Certified Nursing Assistants (CNAs) and other direct care staff in daily resident care
• Administer medications and treatments safely and accurately, following care plans
• Complete timely and accurate documentation in accordance with LTC regulations
• Delegate appropriate tasks to team members while ensuring quality of care
• Communicate changes in residents' condition and treatment effectiveness to the nursing team
• Support household operations and maintain a safe, comfortable environment
• Carry out additional duties as assigned by the Nurse Manager or leadership team

Qualifications and Skills

• Current NYS Licensed Practical Nurse (LPN) license at time of hire
• Current CPR certification (or willingness to obtain)
• New graduates encouraged to apply
• Experience in geriatric, rehabilitation, or long-term care preferred
• Flexibility to meet the needs of residents and the household
• Availability for rotating weekends and holidays
• Strong computer skills and accurate documentation abilities
• Excellent interpersonal and teamwork skills
• Ability to safely perform essential nursing functions (reasonable accommodations considered)

Pay

Pay Range: $30.00-$34.00 per hour, with pay commensurate with license and years of post-license experience

Schedule

Shift differentials: +$1.50/hr for weekday B & C shifts and +10% more/hour for all weekend shifts

Available Shifts

Day (7:00am – 3:00pm), Evening (3:00pm – 11:00pm), Night (11:00pm – 7:00am)

Schedule Options

Full-Time (37.5 hours per week), Part-Time (22.5 hours per week)
