About the Role
As a Licensed Practical Nurse (LPN) at Fairport Rehab & Nursing Center, you will be responsible for providing direct patient care to our residents, including:
- Administering medications and treatments as ordered by physicians
- Assisting with personal care and hygiene needs
- Monitoring vital signs and reporting changes to healthcare team
- Participating in care planning and development of individualized care plans
- Providing emotional support and education to residents and their families
- Collaborating with other healthcare professionals to ensure seamless delivery of care
Responsibilities
- Provide skilled nursing care to residents in accordance with established standards and policies
- Develop and implement individualized care plans that address the unique needs of each resident
- Participate in quality improvement initiatives to improve patient outcomes and satisfaction
- Maintain accurate and detailed records of patient care and treatment
- Communicate effectively with healthcare team members and families to ensure a collaborative approach to care
- Demonstrate a strong commitment to patient-centered care and a passion for working with geriatric populations

Pay
Pay Range: $30.00 – $34.00 per hour
Shift Differentials: +$1.50/hr for weekday B & C shifts and +10% more/hour for all weekend shifts
Sign‑On Bonus: $3000 for Full‑Time LPNs
Schedule
Available Shifts: Day (7:00am – 3:00pm), Evening (3:00pm – 11:00pm), Night (11:00pm‑7:00am)
Schedule Options: Full‑Time (37.5 hours per week), Part‑Time (22.5 hours per week)
Weekend Availability: Required
