About the role
The Licensed Practical Nurse (LPN) at Evergreen Health performs a wide variety of duties and responsibilities in a manner that emphasizes quality of care and customer service. They collaborate with and support team members to ensure a patient-centered approach to care, providing outstanding customer service while maintaining a compassionate, positive, and respectful attitude toward people who use drugs.
Responsibilities
- Collaborates with and supports team members to ensure a patient-focused approach to care
- Provides outstanding customer service while maintaining a compassionate, positive, and respectful attitude toward people who use drugs
- Affirms and implements care for patients within their scope of practice
- Prepares patients for provider visits by taking vital signs, history, and assessing risk factors
- Captures and records health information findings of physical and developmental assessments into the patient record with timeliness and accuracy; accesses and retrieves medical records via HEALTHeLINK™
- Screens all new and returning patients by completing an intake process; comprehensively documents intake encounters and health history into the EMR system
- Facilitates lab and diagnostic tests such as injections, urine, and oral toxicology’s at the request of the provider
Requirements
- Current LPN license
- Current BCLS certification
- Minimum of one (1) year of experience working in a medical practice setting, preferably in an HIV/AIDS or other infectious disease practice
- Prior phlebotomy experience required
Qualifications
- Qualified candidates must be able to communicate clearly and professionally in writing and verbally
- Candidate must demonstrate familiarity with computers and MS Office software (Outlook, Word)
- Demonstrate personal responsibility and integrity and ability to handle Protected Health Information (PHI) in a manner consistent with HIPAA
- Demonstrate awareness of and sensitivity to the issues and health disparities faced by underserved populations is essential
