About the role

Rolling Hills Healthcare is seeking an experienced Licensed Practical Nurse (LPN) to join their skilled nursing facility. The position supports high-quality resident care and fosters a long and successful career.

Responsibilities

• Provide comprehensive Licensed Practical Nurse (LPN) care for residents
• Administer medications and treatments as prescribed by the attending physician and in accordance with facility protocols
• Maintain and monitor residents' health status, reporting changes promptly to supervising RN, physician, and family members
• Deliver skilled nursing services within scope of practice, including tube feedings, suctioning, catheter care, wound care, dressing changes, and other ordered treatments
• Accurately complete required documentation, charting, and reports in compliance with regulatory requirements and facility policies
• Support and implement individualized resident care plans, respecting resident dignity, preferences, and rights
• Observe residents closely, reporting significant changes to the interdisciplinary care team
• Collaborate with the interdisciplinary team to ensure coordinated, high-quality resident care

Requirements

• Active Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) license issued by South Dakota
• BLS certification from the AHA

Qualifications

• Experience in long-term care settings preferred
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team

Skills

• Comprehensive knowledge of licensed practical nurse duties and responsibilities
• Proficiency in administering medications and treatments
• Effective monitoring and reporting of resident health status
• Skilled in delivering and documenting skilled nursing services
• Strong collaboration and teamwork abilities
