Licensed Practical Nurse LPN

About the role

Elkhorn Healthcare and Rehabilitation is seeking an experienced Licensed Practical Nurse (LPN) to join their skilled nursing facility. The position supports the facility's mission of providing high-quality care and fostering a positive reputation in the community.

Responsibilities

• Provide comprehensive Licensed Practical Nurse (LPN) care for residents
• Administer medications and treatments as prescribed by the attending physician and in accordance with facility protocols
• Maintain accurate documentation, charting, and reporting in compliance with regulatory requirements and facility policies
• Support and implement individualized resident care plans while respecting resident dignity, preferences, and rights
• Observe residents closely and report significant changes in condition to the interdisciplinary care team
• Collaborate with the interdisciplinary team to ensure coordinated, high-quality resident care

Requirements

• Active Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) license issued by Montana required
• BLS certification from the AHA required

Qualifications

• Experience in long-term care settings preferred
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team

Skills

• Proficiency in administering medications and treatments
• Knowledge of long-term care protocols and regulations
• Effective problem-solving and critical thinking skills
