About the role

Prairie Heights Healthcare is seeking a Licensed Practical Nurse (LPN) to join their skilled nursing facility in Prairie Heights. The facility is dedicated to providing high-quality care and recognizes the importance of LPNs in achieving this goal.

Responsibilities

• Provide comprehensive Licensed Practical Nurse (LPN) care for residents
• Administer medications and treatments as prescribed by the attending physician and in accordance with facility protocols
• Maintain accurate documentation, charting, and reports in accordance with regulatory requirements and facility policies
• Support and implement individualized resident care plans while ensuring resident dignity, preferences, and rights are respected
• Collaborate with the interdisciplinary team to ensure coordinated, high-quality resident care

Requirements

• Active Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) license issued by South Dakota required
• BLS certification from the AHA required

Qualifications

• Experience in long-term care setting preferred
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
