About the role
Big Horn Rehabilitation and Care Center is seeking a Licensed Practical Nurse (LPN) to join their skilled nursing facility in Sheridan, Wyoming.
Responsibilities
- Provide comprehensive Licensed Practical Nurse (LPN) care for residents
- Administer medications and treatments as prescribed by the attending physician and in accordance with facility protocols
- Maintain accurate documentation, charting, and reports in accordance with regulatory requirements and facility policies
- Support and implement individualized resident care plans while ensuring resident dignity, preferences, and rights are respected
- Observe residents closely and report significant changes in physical, mental, or emotional condition to the interdisciplinary care team
- Collaborate with the interdisciplinary team to ensure coordinated, high-quality resident care
Requirements
- Active Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) license issued by Wyoming required
- BLS certification from the AHA required
Qualifications
- Experience in long-term care preferred
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
