We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our dedicated team in our skilled nursing and long-term care facility, providing high-quality, person-centered care to residents in a safe, supportive environment. Responsibilities

  • Provide essential care to residents, including monitoring vital signs, assisting with daily activities (such as bathing, dressing, and eating), and ensuring proper nutrition and hydration.
  • Administer prescribed medications and treatments, ensuring residents adhere to their medication schedules.
  • Regularly monitor residents' health, observe any changes in their condition, and report these to physicians.
  • Collaborate closely with RNs, physicians, and other healthcare professionals to develop and implement individualized care plans for residents.
  • Supervise Certified Nursing Assistants (CNAs) to ensure quality care delivery.
  • Provide emotional support to residents and their families, addressing concerns and fostering a comforting environment.
  • Communicate with patients' families about their loved ones' health and care plans, ensuring families are informed and involved.
  • Maintain accurate records of each resident's progress and health status to track changes and adjust care plans as necessary.

Requirements

  • Valid LPN license

Schedule

  • 7-7 Day and Night shifts available
  • Sun/M/Tu and every other Wednesday
  • Th/F/Sat and every other Wednesday
