Licensed Practical Nurse (LPN)

About the role

The Licensed Practical Nurse (LPN) plays a vital role in delivering safe, efficient, and patient-centered care across Community Health Net locations. This position includes a $3,000 sign-on bonus, payable in accordance with company policy and contingent upon meeting all employment requirements. LPNs may be required to float between CHN locations based on operational needs.

Responsibilities

• Manage patient flow, including triage, follow-up care, and care coordination
