About the Role

Welcome to Cherokee Park Rehabilitation! As the name implies, we are nestled in the heart of the beautiful Cherokee Park. We are a long-term care and rehabilitation home dedicated to providing exceptional care and support to our residents. We pride ourselves on creating a home-like atmosphere where residents feel valued, comfortable, and respected. Our center features a women-only Alzheimer's care wing, providing specialized and compassionate care in a supportive and safe environment.

Responsibilities

  • Provide bedside care to patients including personal hygiene maintenance
  • Record the patients’ vital signs (including blood pressure, pulse, and respiratory rate)
  • Collect patient’s health information
  • Relay information between patients, registered nurses, and physicians
  • Give oral medications and injections to patients
  • Test a patient’s blood sugar levels
  • Urinary catheter insertion and care
  • Wound care and dressing change
  • Assist patients with personal hygiene
  • Monitor for changes in patients
  • Proper charting, documentation, and maintenance of patient’s medical records
  • Ask questions when in doubt for clarification by the Registered Nurse
  • Acts as a role model for ancillary personnel
  • Maintains the confidentiality of the patient’s clinical records

Requirements

  • Current LPN license in the state
  • At least one year of LPN experience in a long-term care setting is preferred
