About the role
We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to join our team of dedicated healthcare professionals.
Responsibilities
- Provide direct nursing care to residents, including but not limited to assessing their physical and emotional needs, developing individualized care plans, and implementing interventions to achieve optimal outcomes
- Collaborate with other members of the interdisciplinary care team to ensure comprehensive care and services are provided to residents
- Maintain accurate and detailed records of resident care, including treatment plans, progress notes, and incident reports
- Administer medications, treatments, and therapies as ordered by physicians
- Monitor resident conditions and report any changes or concerns to the Director of Nursing or other appropriate staff
- Provide education to residents, families, and caregivers on various aspects of care, including disease management, medication management, and other topics as needed
Requirements
- Active/Current Licensed Practical Nurse (LPN) in the state of Missouri
- Experience in a skilled nursing facility or similar healthcare setting
