Licensed Practical Nurse (LPN)

What You’ll Do

• Provide direct nursing care to residents in accordance with state regulations and facility policies
• Support and guide caregiving staff to ensure consistent, high-quality care
• Assist with resident assessments, care planning, and ongoing monitoring of changes in condition
• Administer medications and treatments per physician orders and established protocols
• Maintain professional relationships with physicians, office staff, pharmacies, and healthcare providers
• Collaborate with the Regional Nurse Consultant and community leadership to support clinical best practices
• Support orientation and training of care team members as needed
• Maintain accurate and timely clinical documentation
• Promote a safe, respectful, and home-like environment for residents

What You Bring

• Education: Graduate of an approved school of nursing
• Licensure / Certification: Current Licensed Practical Nurse (LPN) license required
• Experience & Skills: Experience in assisted living, memory care, or long-term care preferred; Strong clinical judgment and attention to detail; Ability to work collaboratively with residents, families, caregivers, and healthcare partners; Effective communication and teamwork skills; Commitment to senior care and resident advocacy; Alignment with Americare’s mission, values, and culture; Desire to contribute to a positive, caring community environment

Physical Demands

• Lift up to 25 lbs. and assist with resident transfers as needed
• Push, pull, and maneuver up to 100 lbs., including resident mobility assistance
• Maneuver resident wheelchairs weighing up to 350 lbs.
