Licensed Practical Nurse (LPN)

Summary

The Support Nurse (LPN or RN) provides direct resident care and short-term clinical support to Assisted Living communities as assigned. This role supports continuity of care, staff supervision, and resident well-being while ensuring compliance with state regulations, Americare policies, and best practices in person-centered care.

What You’ll Do

• Provide direct nursing care to residents during short-term assignments
• Support a home-like, person-centered care environment
• Supervise and support caregiving staff to meet residents’ physical, psychological, and social needs
• Assist with resident assessments, care planning, and monitoring of changes in condition
• Administer medications and treatments per physician orders and established protocols
• Collaborate with community leadership and senior leadership during assigned support periods
• Maintain accurate and timely clinical documentation
• Promote resident dignity, safety, and quality of life

What You Bring

• Education: Graduate of an approved school of nursing
• Licensure / Certification: Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) license required, maintains current state registration and required continuing education
• Experience & Skills: Experience in assisted living, memory care, or long-term care preferred, Strong clinical judgment and adaptability, Ability to supervise staff and work collaboratively with interdisciplinary teams, Effective communication and interpersonal skills, Commitment to passion for senior care and resident-centered service, Alignment with Americare’s mission, values, and culture, Flexibility to support multiple communities as needed

Physical Demands

• Lift up to 25 lbs. and assist with resident transfers as needed
• Push, pull, and maneuver up to 100 lbs., including resident mobility assistance
• Maneuver resident wheelchairs weighing up to 350 lbs.
