Licensed Practical Nurse (LPN)

This Support Nurse (LPN) role offers short-term assignments across multiple Assisted Living and Memory Care communities, ideal for nurses who adapt quickly, enjoy variety, and want to make a meaningful impact.

Responsibilities

• Provide direct nursing care to residents during short-term assignments
• Support a home-like, person-centered care environment
• Supervise and support caregiving staff to meet residents’ physical, psychological, and social needs
• Assist with resident assessments, care planning, and monitoring of changes in condition
• Administer medications and treatments per physician orders and established protocols
• Collaborate with community leadership and senior leadership during assigned support periods
• Maintain accurate and timely clinical documentation
• Promote resident dignity, safety, and quality of life

Requirements

• Graduate of an approved school of nursing
• Current Licensed Practical Nurse (LPN) license required; maintains current state registration and required continuing education

Skills

• Experience in assisted living, memory care, or long-term care preferred
• Strong clinical judgment and adaptability
• Ability to supervise staff and work collaboratively with interdisciplinary teams
• Effective communication and interpersonal skills

Physical Demands

• Lift up to 25 lbs. and assist with resident transfers as needed
• Push, pull, and maneuver up to 100 lbs., including resident mobility assistance
• Maneuver resident wheelchairs weighing up to 350 lbs.

Pay

$69,000 to $70,000; travel required; generally no on-call requirement; may include occasional overnights.
