About the role

As a Licensed Practical Nurse (LPN) at Sage Bluff Health & Rehab, you will develop meaningful relationships by learning about the life stories of our residents and staff. Responsibilities

  • Promote the physical and psychosocial well-being of residents through nursing skills.
  • Build meaningful relationships with residents and staff by learning their life stories.
  • Collaborate with nursing and facility teams to ensure coordinated and comprehensive care.

Requirements

  • Current and valid Licensed Practical Nurse license in the state of Indiana.
  • Proficient medication management skills.
  • Ability to conduct thorough assessments and accurately document changes in resident condition.
  • Strong passion for geriatric nursing and commitment to senior care excellence.
  • Excellent communication and interpersonal skills.
