Licensed Practical Nurse (LPN)
About the role
Agency: Baptist Health Home Care - New Albany, IN
Schedule: Monday - Friday, 8:00AM - 5:00PM
Territory: Clinicians will work in Washington, Scott, and Clark counties, and the surrounding areas
Responsibilities
- Provide skilled nursing care as ordered by the attending physician and observe and monitor patient conditions.
- Minimize patient risk by administering medication as prescribed by the physician and helping to decrease re-hospitalizations through education on preventative measures and decision-making.
- Educate the patient and family on the disease process using teach back methods to ensure understanding.
- Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
- Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
- Attend in-service training and mandatory agency meetings.
Requirements
- Licensed Practical Nurse (LPN) with current license in the state of employment
- Minimum one-year experience as a LPN in an acute care setting
- Home care experience preferred
- Valid driver's license and auto insurance in your name as a driver
- Capable of all physical demands
