- Minimum of 1 year current LPN experience in an acute care setting (home care experience preferred)
- Authorized to work in the United States
- Valid NYS LPN license
- Certified Basic Life Support (BLS)
- Valid driver’s license (preferred)
- Completed physical exam within the past year
- QuantiFERON test within the past year or chest X-ray within the past five years
- MMR immunizations within the past ten years
