About the role
We are seeking a licensed practical nurse (LPN) to join our hospice team. This position involves conducting patient visits, providing routine care, and communicating with the care team, patients, and their families.
Responsibilities
- Work under the direction and supervision of a registered nurse
- Conduct patient visits and provide routine care
- Communicate with the care team, the patient, and their family
Qualifications
- Graduate of an accredited school of nursing with a current state license as a licensed practical nurse
- 2+ years of nursing experience in an acute care setting, or equivalent experience (hospice experience preferred)
- Reliable transportation
- Ability to sit, stand, bend, move intermittently and lift at least 80-100lbs and bear the weight of an average adult effectively
