Licensed Practical Nurse (LPN) Assisted Living Facility

About the role

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our night shift (11:00 PM – 7:30 AM) team. In this role, you will provide direct nursing care, supervise nursing assistants, and ensure smooth clinical operations within our Supportive Living and Memory Care community.

Responsibilities

• Provide direct nursing care in accordance with physician orders and care plans
• Supervise and guide nursing assistants in daily care delivery
• Administer medications and treatments safely and accurately
• Admit, transfer, and discharge residents as needed
• Complete all required clinical documentation and nurse’s notes
• Receive and transcribe physician orders
• Investigate and report incidents/accidents per policy
• Participate in shift reports and interdisciplinary communication
• Maintain confidentiality of resident information (HIPAA compliance)
• Foster a respectful, safe, and supportive care environment

Requirements

• Current, active Illinois LPN license (in good standing)
• CPR certification required
• Strong clinical judgment and ability to make independent decisions
• Knowledge of nursing practices and Supportive Living regulations
• Prior experience preferred, but not required
• Strong communication and teamwork skills

Physical Requirements

• Ability to move throughout the workday
• Ability to lift, push, pull, or carry up to 25 pounds
• Ability to assist in emergency situations if needed
• Ability to meet the physical, mental, and emotional demands of the role
