About the role
As a Licensed Practical Nurse (LPN) at Onyx Wellness Center, you will provide hands-on nursing care to residents while supporting the clinical and leadership needs of the unit. You will collaborate with nursing staff and interdisciplinary teams to ensure residents receive compassionate, high-quality care.
Responsibilities
- Provide direct nursing care according to individualized care plans
- Administer medications and treatments, ensuring accurate documentation
- Perform clinical assessments and monitor changes in residents’ conditions
- Assist physicians during rounds and exams
- Communicate effectively with residents’ families and care team members
- Ensure compliance with safety, health, and quality regulations
- Foster a respectful, positive, and collaborative team environment
Qualifications
- Current LPN license valid in PA
- Graduate of an accredited LPN or vocational nursing program
- Ability to work independently and collaboratively as part of a care team
- Must pass TB screening and required health checks
- Strong communication, organizational skills, and attention to detail

Pay
$31 – $37 per hour
Schedule
- Shifts: 7 AM to 3 PM | 3 PM to 11 PM | 11 PM to 7 AM
- Full-Time, Part-Time, and Per Diem options available
