We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team. As an LPN you will provide high‑quality care to our residents, including administering medications, observing and reporting changes in residents’ conditions, and participating in care planning. You will work in a fast‑paced, dynamic environment with a dedicated team of healthcare professionals.
Responsibilities
- Provide skilled nursing care to residents, including administering medications, performing vital signs, and documenting patient data.
- Participate in care planning with the interdisciplinary team to develop and implement individualized care plans for each resident.
- Observe and report changes in residents’ conditions to the Registered Nurse and other healthcare professionals.
- Collaborate with other healthcare professionals to ensure the delivery of high‑quality care.
- Maintain accurate and up‑to‑date records of patient care and treatment.
Requirements
- Current LPN licensure in the State of Pennsylvania.
- Strong communication and interpersonal skills.
- Ability to work effectively in a team environment.
