About the Role
As the Homecare Licensed Practical Nurse (LPN), you will perform selected nursing care for patients in Care of Sick and/or Hospice Programs as delegated by the Primary Care Nurse (PCN), working within a nursing team under the general supervision of the Clinical Supervisor.
Responsibilities
- Collaborate with the PCN to review the Plan of Care and provide nursing care to patients in their homes on a family-centered basis while recording and maintaining complete, accurate patient records on a Portable Electronic Device and/or via paper record.
- Make patient assessments, report, and document appropriately as delegated by the registered nurse within your scope of practice.
- Work under the direction of the PCN to orient and delegate responsibility to Home Health Aides within regulatory guidelines and agency policy.
Requirements
- LPN Degree required.
- Current Connecticut LPN license required.
- Minimum of four (4) years of nursing practice, with at least two (2) years of recent Med-Surg, acute care, or critical care experience as a Licensed Practical Nurse preferred.
- Valid Connecticut Driver’s License and proof of current auto insurance coverage required.
