As a Licensed Practical Nurse (LPN) in our outpatient gastroenterology practice, you'll play a vital role in connecting patients with the care they need while serving as a trusted clinical resource throughout their treatment experience. In this dynamic role, you'll work alongside a collaborative team of physicians and advanced practice providers to coordinate patient care, communicate test results and treatment plans, answer clinical questions, and guide patients through procedures and follow-up care. Responsibilities
- Serve as the primary clinical liaison between patients, providers, and ancillary departments to ensure seamless coordination of care.
- Communicate laboratory, pathology, imaging, and procedure results to patients as directed by the provider.
- Review and explain individualized treatment plans, provider recommendations, medication instructions, and follow-up care.
- Respond to patient phone calls, electronic messages, and patient portal inquiries in a timely, professional, and compassionate manner.
- Triage patient concerns using established clinical protocols and escalate urgent or complex issues to the appropriate provider.
- Schedule endoscopic procedures, diagnostic testing, imaging, referrals, and follow-up appointments while ensuring all clinical requirements are met.
- Provide comprehensive pre-procedure and post-procedure education, including bowel preparation instructions, medication management, dietary restrictions, and recovery expectations.
- Coordinate medication refills in accordance with provider orders and established practice protocols.
- Monitor outstanding laboratory, pathology, imaging, and procedure results to ensure timely provider review and patient notification.
- Carefully coordinate care with primary care providers, specialists, pharmacies, infusion centers, hospitals, and other healthcare partners.
- Maintain accurate, thorough, and timely documentation of all patient interactions, clinical communications, and care coordination activities within the electronic medical record.
- Review patient charts for completeness and identify outstanding clinical needs prior to appointments and procedures.
- Educate patients and caregivers on gastroenterology conditions, prescribed therapies, lifestyle modifications, and disease management strategies.
- Maintain compliance with HIPAA, OSHA, state nursing regulations, and organizational policies and procedures.
- Foster a positive, collaborative work environment by communicating effectively with physicians, advanced practice providers, clinical staff, and administrative team members.
Requirements
- Education & Licensure: Graduate of an accredited Practical Nursing program. Current, unrestricted Licensed Practical Nurse (LPN) license in the Commonwealth of Virginia. Current Basic Life Support (BLS) certification required.
- Experience: Minimum of one (1) year of clinical nursing experience preferred. Previous experience in gastroenterology, ambulatory care, internal medicine, family practice, or another outpatient specialty preferred. Experience using electronic medical records (EMR); ModMed gGastro experience is a plus.
- Knowledge, Skills & Abilities: Excellent verbal and written communication skills with the ability to educate patients and caregivers effectively. Strong critical thinking, clinical judgment, and problem-solving abilities. Demonstrated ability to triage patient concerns and prioritize competing demands in a fast-paced environment. Exceptional organizational and time management skills with close attention to detail. Ability to manage multiple tasks while maintaining accuracy and efficiency. Strong customer service skills with a commitment to providing compassionate, patient-centered care. Ability to work collaboratively with physicians, advanced practice providers, nurses, and administrative staff. Knowledge of HIPAA regulations and patient confidentiality requirements. Proficient in Microsoft Office applications and electronic medical record systems. Ability to adapt to changing priorities, workflows, and technology.
