Licensed Practical Nurse – Long-Term Care & Rehab

About the role

We are seeking a compassionate Licensed Practical Nurse (LPN) to deliver high-quality care to residents in a rehabilitation and nursing facility.

Responsibilities

• Administering medications and treatments
• Monitoring resident conditions and documenting accurately
• Collaborating with RNs, therapists, and physicians to develop and support individualized care plans
• Educating residents and their families about care and health management
• Supporting activities of daily living (ADLs) and maintaining a safe, respectful environment

Qualifications

• Valid LPN license
• Strong communication and teamwork skills
• Compassionate, detail-oriented, and committed to person-centered care
