About the role
This position is open to current Colorado residents only. The Colorado State Veterans Community Living Center at Rifle is located in the scenic Colorado River valley, offering skilled nursing care and support for veterans, their spouses, and eligible "Gold Star" parents. The center is committed to providing a life of quality and dignity for its residents.
Responsibilities
- Performs both admission and daily assessments during clinical stays, identifying critical incidents and emergencies.
- Identifies and implements appropriate nursing interventions and treatment plans under RN oversight.
- Documents care and treatment provided accurately and pertinently, reporting specific changes in resident’s health status.
- Administers medications correctly, ensuring adherence to the five rights of safe medication administration and resident rights.
- Ensures compliance with nursing standards, policies, and regulations, and reviews physician orders and care plans.
- Provides compassionate care and maintains confidentiality of resident information.
Requirements
- Current, valid licensure as an LPN from the Colorado Board of Nursing or an eNLC participating state.
- Ability to follow patient confidentiality standards and provide compassionate care.
- Excellent interpersonal and communication skills.
- Ability to read and comprehend the English language.
- Ability to communicate fluently in English both verbally and in writing.
- Full background check, including CBI & FBI fingerprint check, Colorado Court Database check, Medicare Fraud Database, TRAILS Database check, and CAPS check.
- Valid, non-restricted Colorado Driver’s License or a non-restricted U.S. Driver's License for positions within 30 miles of the CO state border.
- Completion and passing of TB test and any other vaccinations as required by the facility.
Qualifications
- Relevant years of state service experience.
- Minimum qualifications: Current, valid LPN license, ability to follow patient confidentiality standards, excellent interpersonal and communication skills, ability to read and comprehend the English language, ability to communicate fluently in English both verbally and in writing, full background check, valid, non-restricted Colorado Driver’s License or a non-restricted U.S. Driver's License for positions within 30 miles of the CO state border, completion and passing of TB test and any other vaccinations as required by the facility.
