Licensed Practical Nurse - Hospice

Responsibilities

• Provide direct patient care, utilizing various skills and coordinating services
• Complete documentation per policies and regulations
• Follow care plans developed by the Registered Nurse
• Coordinate hospice services with the healthcare team, patients, families, and community agencies
• Accept and transcribe physician's orders
• Forward clinical notes to the Registered Nurse for care coordination
• Participate in interdisciplinary team meetings

Qualifications & Skills

• Graduate from an approved nursing program
• Current LPN licensure in the state
• Minimum one year of clinical nursing experience, preferably in acute care
• Ability to organize work independently and deliver effective care
• Strong communication skills (oral and written)
• CPR certification
