About the role
Licensed Practical Nurse (LPN) providing patient-centered care in home settings for pediatric and adult patients with G-tube, Trach, and Ventilator needs.
Responsibilities
- Assess, plan, implement, and evaluate patient care using the nursing process.
- Deliver skilled nursing services according to prescribed orders.
- Monitor signs and symptoms to identify physiological and psychosocial changes.
- Collect, analyze, and interpret healthcare data to identify nursing diagnoses.
Qualifications & Skills
- Licensed as an LPN/LVN in the practicing state.
- Current TB or Chest X-Ray and BLS certification.
Key Duties
- Provide safe, compassionate, and patient-centered care in home environments.
- Ensure adherence to medical orders.
- Assess patient needs to promote health and well-being.
