About the role
Trilogy Home Health, a Humana Company, is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide high‑quality care to home health patients in their home or facility, working closely with physicians, RNs, and the clinical team to improve patient outcomes.
Location & Employment Type
Naples/Collier County, FL – Full‑time position.
Pay
Salary: $30 – $35 per hour (depending on experience) plus mileage reimbursement.

Qualifications
- Licensed Practical Nurse (LPN) license in the state of Florida
- Graduate of an accredited nursing program
- Minimum 1 year of nursing experience (home health experience preferred)
- IV certification
- Strong initiative and independent judgment
