Licensed Practical Nurse (LPN)

BAYADA Home Health Care is seeking a compassionate and dedicated per-diem Licensed Practical Nurse (LPN) to serve clients in Davidson and Williamson Counties, Tennessee. The position involves making home visits, administering medications, performing wound care, and following doctor's orders. Responsibilities also include monitoring clients, documenting care, and coordinating with other healthcare professionals.

Responsibilities

• Making home visits to clients in designated territories.
• Performing assigned duties such as administering medication, wound care, treatments, and procedures.
• Monitoring clients' conditions and reporting changes to Clinical Manager and Client Services Manager.
• Following up with clients, executing and documenting doctors' orders.
• Performing client assessments as necessary.
• Case management and coordination.
• Accurately documenting observations, interventions, and evaluations pertaining to client care management and services provided, using electronic medical records on a touch pad tablet.

Qualifications

• A current license as a Licensed Practice Nurse in the state of TN.
• A minimum of one year of recent, verifiable experience as an LPN.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Prior home care experience strongly preferred, but not required.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.
