Licensed Practical Nurse- Home Health Visits

About the role

BAYADA Home Health Care is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care to adult clients in their homes throughout Northampton County on a per-visit basis. As a home care nurse, you will be an integral member of a multidisciplinary healthcare team, delivering skilled nursing and rehabilitative care that enables clients to receive necessary medical care while remaining at home.

Responsibilities

• Make home visits to clients in designated territories
• Administer medications, perform wound care, and carry out treatments and procedures as assigned
• Monitor clients' conditions and report changes to the Clinical Manager and Client Services Manager
• Follow up with, execute, and properly document physicians' orders
• Perform client assessments as necessary
• Provide case management and care coordination
• Accurately document observations, interventions, and evaluations using electronic medical records on a state-of-the-art touch pad tablet

Requirements

• Current license as a Licensed Practical Nurse in Pennsylvania
• Minimum one year of recent, verifiable experience as an LPN
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
• Ability to work independently and manage time effectively
• Strong interpersonal skills
• Solid computer skills
• Ability to travel to cases as assigned

Preferred Qualifications

• Prior home care experience
• Prior experience with electronic medical records (EMR)

Pay

Base pay: $35.00 to $45.00 per visit, depending on qualifications and subject to negotiation
