Licensed Practical Nurse - Home Health

About the role

We are seeking a full-time Home Health LPN to provide compassionate, person-centered nursing care in the greater Lebanon Ohio area, including Lebanon, Middletown, Springboro, Dayton, and Cincinnati.

Responsibilities

• Assist with a primary caseload of skilled home health patients, delivering care as prescribed by physicians and in compliance with Medicare, state laws, and agency policies.
• Provide professional nursing services to patients, families, and caregivers, ensuring high-quality, personalized support.

Requirements

• Must have a valid Ohio LPN license.
• CPR certification required.
• At least 2 years of experience in hospice or home care.

• Licensed driver with a reliable vehicle.

Pay

Compensation is commensurate with experience.

Schedule

Monday to Friday, 8 am to 5 pm, with some on-call responsibilities.
