Licensed Practical Nurse - Home Care

About the role

We are seeking a Licensed Practical Nurse (LPN) to provide in-home personal care services in designated counties. This role involves delivering high-quality nursing care aligned with patient care plans, reviewing and following plans before visits, and communicating updates or concerns.

Responsibilities

• Deliver high-quality nursing care in accordance with patient care plans
• Review and follow care plans before each visit
• Communicate updates or concerns regarding patient care
• Manage medications and provide patient and family education
• Identify safety issues in the home environment
• Collaborate with care team members, including RN case managers and physicians

Requirements

• Active LPN/LVN license in Missouri
• At least one year of acute care experience (home health preferred)
• Strong communication, organization, and healthcare knowledge
• Reliable transportation
• Ability to meet health and compliance requirements
