Licensed Practical Nurse - Full-Time

Responsibilities

• Provide direct patient care including medication administration, wound care, vital signs monitoring, and disease management education in clients' homes.
• Collaborate with RNs, therapists, and families to develop and implement personalized care plans that ensure safety, comfort, and independence.
