Licensed Practical Nurse (Full-Time)
Thursday - Sunday, 10-hour shifts in the Littleton area.
About the role
As an LPN at NCHHHA, you will get to know your individual patients and support your team members in executing tailored plans of care for Home Health and Hospice patients. You will perform comprehensive assessments and provide skilled nursing care for each patient under the supervision of and in partnership with a qualified Registered Nurse. Our LPNs are an integral part of a dynamic, multi-disciplinary team. You will maintain a high level of autonomy and work to the top of your license. You will enjoy a flexible workday, with an option to start and end your day from home. Strong clinical support and educational resources are provided, as well as support in obtaining certifications in your area of expertise.
Responsibilities
- Perform comprehensive assessments of Home Health and Hospice patients.
- Provide skilled nursing care under the supervision of and in partnership with a Registered Nurse.
- Execute tailored plans of care for individual patients.
- Function autonomously while working to the top of your license.
- Demonstrate flexibility in response to unexpected changes in work volume and schedule.
- Function in any home situation regardless of patient demographics or financial condition.
- Interact professionally as a member of a multi-disciplinary team.
- Perform data entry in electronic medical records.
Requirements
- Current active New Hampshire LPN License.
- Minimum of 1 year of experience in a healthcare setting.
- Ability to speak, write, read, and comprehend English.
- Computer proficiency in electronic medical records data entry.
- Valid driver’s license and certificate of auto insurance coverage.
- Current BLS Certification.

Orientation
All new hires are required to attend a mandatory three-day orientation at Weeks Medical Center in Lancaster, NH. This orientation provides essential training, compliance information, and an introduction to our organizational culture to ensure a smooth and successful transition into your role. Attendance on all three days is required for onboarding completion. If Weeks Medical Center is not your designated home base, you will be eligible for travel reimbursement in accordance with our standard travel policy.
