Responsibilities

  • Provide direct and indirect patient care under the direction of a Registered Professional Nurse, Nurse Practitioner, Physician's Assistant, or Physician as part of an interdisciplinary healthcare team.
  • Collect and record accurate patient health data with minimal exceptions (3-5 or fewer).
  • Evaluate health data to identify patient problems with minimal exceptions (2-3 or fewer).
  • Implement and document specified nursing interventions with minimal exceptions (3-5 or fewer).
  • Identify deviations from expected patient outcomes with minimal exceptions (2-3 or fewer).
  • Adhere to approved, research-based practice standards with minimal exceptions (2-3 or fewer).
  • Perform and document Point of Care (POC) testing and quality control according to laboratory policies, with no more than one exception per year.
  • Maintain and document testing reagents, supplies, and equipment with no more than one exception per year.
  • Advocate for patients to preserve their autonomy, dignity, and rights.
  • Deliver care in a non-judgmental, non-discriminatory manner sensitive to patient diversity.
  • Ensure patients have informed consent signed for indicated procedures.
  • Maintain current licensure, certification, and competencies, including BLS validation.
  • Complete required unit credentialing and annual training as per departmental policy.
  • Practice within the New York State-defined LPN scope of practice 100% of the time.
  • Acquire and maintain current knowledge and skills in your area of practice.
  • Participate in annual educational activities related to job function (4-8 hours for LPN Ambulatory Care; 8-12 hours for LPN Ambulatory Care Advanced).
  • LPN Ambulatory Care Advanced only: Serve as a resource for team members, assist in orienting new staff, direct and supervise care provided by other LPNs and support personnel, and demonstrate operational problem-solving ability.
  • Perform home visits if required.
  • Perform other duties as requested or directed by supervisor.

Requirements

  • Graduation from a Practical Nursing program.
  • Current NYS licensure as a Practical Nurse or valid interim permit.
  • BLS certification within 6 months of hire.
  • Additional credentials as determined by the specific work unit.
  • Previous healthcare experience preferred.

Physical Requirements

  • Light work: Exerting up to 20 lbs of force frequently and/or negligible force constantly to move objects.
  • Occasional: Balancing, crouching, kneeling, typing/clerical/dexterity, grasping, lifting, pulling, pushing, reaching, stooping.
  • Frequent: Hearing, seeing/monitor/computer use, standing, talking, visual acuity (color, depth, perception), walking.
  • Hazards: Frequent exposure to bodily fluids/bloodborne pathogens.
  • Working conditions: Occasional odors, frequent noise.

Travel

Occasional travel may be required, but not on a regular basis.

Pay

Salary is based on qualifications, experience, education, licenses, specialty, training, and industry standards. Pay range: $24.11–$34.84 per hour.
