Licensed Practical Nurse for Personal Care Management Program
About the role
As a Licensed Practical Nurse (LPN) in the Personal Care Attendant (PCA) Evaluation Department, you will work in a unique program that makes a meaningful difference in the lives of children and adults. You will set your own schedule and conduct reevaluations and adjustments for the MassHealth PCA program, working with individuals with a wide range of disabilities.
Responsibilities
- Perform annual reevaluations of members applying for personal care services in accordance with MassHealth regulations 130 CMR 422.422(C).
- Submit completed evaluations within the specified timeframe.
- Respond to hours adjustment requests in a timely manner.
- Address MassHealth inquiries and provide professional input regarding PCA evaluations as requested.
- Learn and adhere to PCA Program standards.
- Schedule reevaluation visits and update your calendar accordingly.
- Document progress notes in the database in a timely manner.
- Communicate with PCA Evaluation Department Assistants regarding no-shows, no-contacts, hospitalizations, termination requests, and other concerns.
- Submit monthly mileage sheets within the required timeframe.
- Work collaboratively with other members of the PCA Program.
- Communicate with your Supervisor and Office Staff when issues arise.
- Interact with Tempus Unlimited, Inc. offices to resolve concerns related to evaluations in support of consumer independence.
- Attend monthly staff meetings.
- Report suspicion of fraud to the PCM Management Team.
- Report all suspected abuse or neglect to the appropriate agency, such as DPPC, Executive Office of Elder Affairs, or Department of Children and Families.
- Represent the agency in a professional, positive manner and provide culturally sensitive services.
- Perform other duties as assigned by the Manager.
Requirements
- Current Licensed Practical Nurse (LPN) license to practice in Massachusetts in good standing with the Board.
- At least 2 years of recent experience in the direct care of the elderly or disabled population.
- Ability to establish and maintain professional relationships with consumers, family members, co-workers, and community professionals.
- Ability to independently organize work and meet deadlines.
- Ability to multi-task in a busy environment.
- Excellent communication skills.
- Accuracy and attention to detail.
- Ability to adapt to constant change with a positive attitude.
- Ability to maintain confidentiality.
- Ability to work efficiently as a team player.
- Ability to exercise sound judgment in decision-making.
- Fluency in other languages is preferred.
- Familiarity with the MassHealth PCA Program is preferred.
Schedule
- Full-time, Monday - Friday.
- Core hours: 8:30 AM to 4:30 PM.
- 2-day in-person orientation required at the Stoughton office.
Pay
$31 per hour.
Mileage reimbursement provided.
Work Environment
This job operates in both a professional office and a home office environment. Routine use of standard office equipment such as computers, phones, photocopiers, and electronic filing systems is required.
Physical Demands
While performing the duties of this job, the employee is frequently required to sit, stand, bend, stoop, walk, carry, and lift objects. Travel within the coverage area is required, and access to reliable transportation is necessary for community visits.
