Responsible For
- Provides direct patient care as defined in the LPN State Nurse Practice Act
- Implements current nursing practice following a patient assessment and an approved Plan of Care initiated by the Hospice Services Registered Nurse.
- Provides accurate and timely documentation of Hospice patient services to reflect the Plan of Care.
- Affords Hospice Services patient/family/caregiver education and information pertinent to diagnosis and Plan of Care.
- Participates in coordination of agency services, appropriately reporting the identified needs for agency Interdisciplinary Group members.
- Uses equipment and supplies effectively and efficiently.
- Provides appropriate pain/symptom management; evaluates Hospice Services patient’s response(s) to treatments/medications.
- Participates in personal and professional growth and development.
- Promotes the image and reputation of the System by exhibiting servant leadership and providing direct and open lines of communication.
- Contributes to the work of committees, workgroups, project management, and other collaborative efforts of the System.
Qualifications
- Nursing degree from an accredited college or university or is a graduate of an approved LPN program
- Current license / registration by state of hire as a LPN
- Current CPR certification
