Who We're Seeking
We're seeking a Licensed Practical Nurse (LPN) to join our Hospice team. Your role involves communicating effectively and promptly with all members of the healthcare team and maintaining accurate electronic documentation of care provided. You collaborate with patients, nurse case managers, physicians, and families to implement plans of care within State and Federal regulations and CVHHH policies.
Responsibilities
- Determine patient care needs and carry out goal-oriented plans of care.
- Ensure accurate and timely documentation of patient information, adhering to HIPAA regulations.
- Work autonomously under RN direction and utilize supervisory support when necessary.
- Empower clients to problem-solve by exploring options of care, when available and appropriate, and alternative plans, when necessary, to achieve desired outcomes.
- Participate in on-call rotation and respond to calls promptly.

Requirements
- Valid Vermont LPN License.
- Minimum of one year progressive clinical experience. Hospice experience preferred.
- Valid Vermont Driver’s License.
