About the role

Under the supervision of a Registered Nurse, and in collaboration with the Interdisciplinary Team (IDT), provides direct and indirect patient care in accordance with the Nurse Practice Act and organizational policy.

Responsibilities

  • Positively collaborates in the development and modification of the Field Staff Nurse/Case Manager comprehensive nursing plan of care for the hospice patient.
  • Performs physical assessment and collects data then effectively collaborates with the Field Staff Nurse/Case Manager in the assessment of the health status of a patient.
  • Implements aspects of the plan of care, as delegated.
  • Participates in health teaching and counseling to promote, attain, and maintain the optimum health level of patients, as delegated.
  • Serves as an advocate for the patient/family/caregiver by communicating and collaborating with the IDT, as delegated.
  • Participates in the evaluation of patient responses to interventions.
  • Provides direct patient care.
  • Participates in on-call after-hour care as needed and in the absence of assigned on-call staff; in the on-call role, the LPN triages calls and provides care under the direction of a Registered Nurse.
  • Demonstrates basic LPN clinical skills and gives a practical exhibition and explanation at the required annual skills day.
  • Provides effective, ongoing patient/family/caregiver education regarding patterns of disease progression at the end-of-life, as delegated.
  • Practices within the guidelines of the Illinois Department of Professional Regulation.
  • Provides evidence-based physical, emotional, psychosocial, and spiritual care under the direction of a Registered Nurse.
  • Demonstrates knowledge of the principles of end-of-life pain and symptom management.
  • Participates in monitoring symptoms and changes.

Pay

$28.72 Hourly. Full-Time Non-Exempt Position

Schedule

Monday–Friday 8:30 AM–5:00 PM with an additional rotational on-call schedule that includes some evening, weekends, and holidays. On-call is limited due to the presence of assigned on-call nurses.

Location

Based out of the Belleville Patient Services Office. Primary service area includes the northern territory covering Clinton, Madison, Bond, Montgomery, Macoupin, Jersey, and Calhoun counties. Travel required to patient residences, long-term care facilities, nursing homes, hospitals, etc.

Requirements: valid driver’s license, current automobile insurance, and reliable transportation. Company reimburses .70 cents per mile for driving personal vehicles.
