LICENSED PRACTICAL NURSE - FLOATING

About the role

Float LPN provides flexible staffing across various program settings, ensuring high-quality patient care and adapting to diverse clinical environments. Under Nursing supervision, provides medication management and offers general health and wellness support. Works collaboratively with other providers in the residence to facilitate client wellness.

Responsibilities

  • Provides medication management within the residential setting
  • Oversees the residential self-medication process for clients
  • Documents the medication self-administration process in each client’s EMAR
  • Tracks client compliance with required medications, identifying any concerns to other Prescribers and other medical staff
  • Reports medication compliance to the clinical team
  • Communicates verbally with NSM and residential staff when calling off for unplanned absences
  • Supports the development of client wellness education and activities
  • Affords support with vital signs and other assistance to providers as needed
  • Collaborates with appropriate residential staff on client discharge plans
  • Participates in staff meetings and medical/clinical supervision as directed
  • Participates in relevant Agency meetings and/or trainings
  • Actively pursues development of professional competencies related to job role performance through reading, supervision, in-service training and attendance at conferences and workshops

Qualifications

  • Education: Minimum of a High School Diploma or GED and accredited LPN diploma required.
  • Credentials: Valid NYS Licensed Practical Nurse required.
  • Experience: Two to three years of experience related to nursing services to individuals with mental health, and chemical dependency / substance use disorder related services preferred.
  • Bi-lingual in Spanish preferred.
