Responsibilities
- Joint (Floor and Phone) Medical Home
- Determines patient needs and reports back to the provider.
- Works with Health Coaches to schedule appointments.
- Educates/instructs patients and families on treatment and medication.
- Prepares exam rooms and equipment.
- Assists providers with retrieving and obtaining medical records and lab work.
- Dispenses medications at the direction of a provider.
- Handles prescription renewals and inquiries.
- Provides test results.
- Performs phlebotomy and specimen collection.
- Completes order entry and documentation.
- Answers and returns phone calls.
- Provides pharmacy refills.
Requirements
- Graduate of an accredited school of nursing.
- Previous Licensed Practical Nurse (LPN) or medical experience preferred.
- Phlebotomy experience required.
- Current Licensed Practical Nurse (LPN) License, valid compact multistate license or temporary permit required.
- Knowledge of medical terminology and procedures.
- Ability to manage time and prioritize tasks.
- Effective communication skills.
- Physical demands: Medium work, occasionally lifting up to 50 pounds, frequently sitting, standing, walking, and bending.

- Knowledge of medical terminology and procedures.
- Knowledge of medicine including recognizing and diagnosing diseases.
- Time management and prioritization skills.
- Quick typing and information capturing abilities.
