Responsibilities

• Joint (Floor and Phone) Medical Home
• Determines patient needs and reports back to the provider.
• Works with Health Coaches to schedule appointments.
• Educates/instructs patients and families on treatment and medication.
• Prepares exam rooms and equipment.
• Assists providers with retrieving and obtaining medical records and lab work.
• Dispenses medications at the direction of a provider.
• Handles prescription renewals and inquiries.
• Provides test results.
• Performs phlebotomy and specimen collection.
• Completes order entry and documentation.
• Answers and returns phone calls.
• Provides pharmacy refills.

Requirements

• Graduate of an accredited school of nursing.
• Previous Licensed Practical Nurse (LPN) or medical experience preferred.
• Phlebotomy experience required.
• Current Licensed Practical Nurse (LPN) License, valid compact multistate license or temporary permit required.
• Knowledge of medical terminology and procedures.
• Ability to manage time and prioritize tasks.
• Effective communication skills.
• Physical demands: Medium work, occasionally lifting up to 50 pounds, frequently sitting, standing, walking, and bending.
