Licensed Practical Nurse - Family Practice - Part Time

About the role

This Licensed Practical Nurse (LPN) position assists physicians by reviewing records, preparing patients and exam rooms, and handling prescription renewals and inquiries. The role operates within a Joint (Floor and Phone) Medical Home model.

Responsibilities

• Documents all lab results, procedures, and appointments on tracking forms.
• Completes tracking forms and follows up on missed appointments and procedures.
• Assists Health Coaches by working patient lists and scheduling appointments as directed.
• Provides patient and family education at the direction of the provider to ensure knowledge and awareness, recommending community resources as needed.
• Educates and instructs patients according to provider direction 98% of the time.
• Performs telephone triage to determine patient needs, recording and reporting patient condition to the provider, and advising patients based on provider instructions.
• Returns patient calls within 90 minutes based on medical priority 95% of the time.
• Accurately records and reports patient conditions according to department policy 98% of the time.
• Maintains clean and organized work areas, equipment, and exam rooms according to department and provider protocols 99% of the time.
• Handles prescription renewals and inquiries, discussing questions with providers and relaying information to pharmacies according to protocols 99% of the time.
• Provides age-specific care to ensure patient understanding and comfort.

Floor Duties

• Prepares exam rooms and equipment by setting up instruments, stocking supplies, and organizing according to provider protocol 95% of the time.
• Prepares patients for exams by reviewing medical records, checking vital signs, performing medication reconciliation, and obtaining relevant patient information according to protocol 95% of the time.
• Assists providers by retrieving tests, medical records, and obtaining lab work as directed 95% of the time.
• Dispenses medications at the direction of a provider, ensuring patients and family members understand how to take medications correctly.
• Performs phlebotomy with proper labeling and handling of specimens, completes requisitions, and collects specimens according to standard procedures and protocols 95% of the time.
• Ensures accuracy of order entry and completes appropriate documentation without error at least 98% of the time.

Phone Duties

• Answers phone calls according to protocol at least 98% of the time.
• Returns phone messages according to protocol at least 98% of the time.
• Provides test results according to protocol at least 98% of the time.
• Accurately completes pharmacy refill requests 100% of the time.

Requirements

• Graduate of an accredited school of nursing.
• Current valid Licensed Practical Nurse (LPN) License, valid compact multistate license, or temporary permit while awaiting licensure.
• Current American Heart Association or American Red Cross Basic Life Support (BLS) strongly preferred at time of hire, required within 3 months of hire.
• Previous LPN or medical experience preferred.
• Phlebotomy experience required for floor duties.

Monday and Wednesday, 7:00 AM - 5:30 PM.
