Licensed Practical Nurse - Clinic
Primary Responsibilities
- Triage Patients
- Supports Quality Initiatives
- Maintains Nursing Appointment Schedule
- Follows Up Post-Hospitalized or Post-Emergency Room Visits
- Screens Patients for Care Opportunities
- Develops Care Plans with Treating Physician
- Provides Patient Education and Anticipatory Guidance
- Coordinates Care Coordination
- Administers IV Therapy and Prescribed Treatment
- Maintains Clinical Documentation
Requirements
- Active LPN License
- BLS Certification
- Clinic Experience Preferred
Qualifications
Active LPN license, BLS certification, clinic experience preferred.
